Can the chain mount be eliminated?

Yes, this can be a ceiling mount or chain hung.

is 15.25 the full height (not including chain)? What is the width?

The Progress Lighting P3645 is 15.25 inches tall not including any chain. It is 9 inches wide. I hope this helps!

We need a light that is at least 5 inches in diameter to cover the hole in the ceiling where the light we have now is. Is this going to cover the 5 inch hole?

The Progress Lighting P3645 has a 5.5" canopy. If your hole is 5" it should be enough to cover it.

Is the bottom open for easy access to change bulb?

The Progress Lighting P3645 has clips on the top and bottom of the frame that allowing you to take out one of the panels which will then allow you to change out the lights.

Is the glass beveled?

The Progress Lighting P3645 glass is a clear flat glass, it is not beveled.
