Is all the hardware provided (base, etc) to attach this pole to a concrete floor?

The Progress Lighting P5390 has been discontinued and there is no stock available.

Does the wiring come out from the base of the post?

Yes, the Progress P5390 has the wiring exiting the base of the post.
